Building effective digital post-hospital discharge and reablement services for older adults in social care: a mixed methods process evaluation using the NASSS framework

Funded by National Institute for Health Research.

Collaboration with Age UK Bristol, Bristol Council, King's College London, London School of Economics, Middlesbrough Council, Newcastle University, Northumbria University, Oxfordshire Council, The Open University and University of Hertfordshire.

This project will conduct a process evaluation of factors influencing delivery, implementation and adoption of digital technologies within post hospital discharge services for older adults. The application is an output of the DIALOGUE project development grant awarded in February 2024 (Worktribe ID 1906129). The submission will be to the NIHR's Research Programme for Social Care - Highlight notice.

The project will carry out a qualitative evaluation of factors influencing local adoption, uptake, implementation and abandonment of post hospital discharge services for older people, using digital technologies and delivered within primary care. The project will also include a cost effectiveness assessment for these services

The project will include the following academic partner Universities; Northumbria, Newcastle, Open University, Bristol, London School of Economics, King's College London.
